The Deer Park Dental, Ltd. Membership Plan is an annual membership plan that allows individuals and families who don’t have Dental Insurance to receive quality dental care at a reduced cost. To ensure a stress free visit we’ve created an alternative to dental insurance. With our in office membership plan you’ll be put at ease that you are getting the best care at the best price. Adult Plan

For patients with regular cleanings and no periodontal disease.
- Routine cleanings (two per year)
- Routine oral exams (two per year)
- One set of 4 bitewing x-rays
- One emergency exam per year with necessary x-rays, if needed
- One set of full mouth x-rays or panoramic x-ray
- Periodontal charting
- Intraoral photos
- Oral cancer screenings
Kids Plan
/ year
For patients age 12 years and younger.
- Routine cleanings (two per year)
- Routine oral exams (two per year)
- One set of 4 bitewing x-rays
- One emergency exam per year with necessary x-rays, if needed
- One panoramic x-ray
- Fluoride treatment (two per year)
- Periodontal charting
- Intraoral photos
- Oral cancer screenings
Periodontal Plan
/ year
For patients with periodontal (gum) disease.
- Four periodontal maintenance cleanings
- Routine oral exams (two per year)
- One set of 4 bitewing x-rays
- One emergency exam per year with necessary x-rays, if needed
- One set of full mouth x-rays or panoramic x-ray
- Periodontal charting
- Intraoral photos
- Oral cancer screenings
Plan Provisions:
- This plan is only valid at Deer Park Dental, Ltd.. Not valid at any specialist’s office Deer Park Dental, Ltd. may refer to.
- This plan is a yearly membership plan.
- Membership plan effective the day you enroll & plan must be paid for on enrollment date.
- Premiums are non-refundable. No Refunds will be given if participant decides not to utilize the dental savings program.
- Any patient treatment fee is due at time of service. Not valid in conjunction with Dental Insurance or any other fee schedule.
- Membership plan is non-transferable to another patient.
